Registered Care Services

Hexagon manages two registered care homes and the services provided are reviewed by the Care Quality Commission. These are Townley Road in Southwark (CQC report) and Woodcote Road in Sutton (CQC report). These services provide intensive rehabilitation for people with complex mental health needs. The focus is on rehabilitation and the homes are run along domestic lines, looking and feeling like homes.

The residents have often had unsuccessful community placements or extended periods of time in hospital and the staff team of psychiatric nurses and support workers work closely with the residents and the local Community Mental Health Teams (CMHT).

Using the Recovery Model, the aim is for residents to develop confidence, improve daily living skills and move on to lower support after about two years.  Most residents take up activities outside the home with some achieving success in training courses and part-time work.

Team Leaders [Registered Mental Health Nurses] and Support Workers provide individual rehabilitation programmes that are agreed with residents. The Recovery Model underpins our work which aims to fulfil individual’s hopes and aspirations through:

• Empowerment, self- esteem and confidence building
• Meaningful activities -training, education, volunteering, work
• Taking control of medication & treatment
• Developing activities of daily living
• Emotional & physical support
